ASHTABULA — Sally A. Dulak, 69, of Burlingham Drive, died Monday, Nov. 19, 2012, at Austinburg Nursing and Rehabilitation Center from complications of Parkinson’s disease.
Arrangements are incomplete and will be announced by Zaback-Williams-Ducro Funeral Home, 500 W. Prospect Road, Ashtabula.
